# How to publish a Form

**A form on a website is a powerfull function to get user feedback, to get newsletter subscriptions, register for downloads and much more.**

We have three types of forms which can be embedded in a post or page on a website. Make sure that you use these forms as your starting point when you create a new form.

These forms a redesigned with several **objectives** in mind.

1. The name and address information is defined in such a way that this content can be imported to our Dynamics CRM system for further processing
2. It is not allowed to ask data from the client other then that is needed to fullfill the call to action. For example, in a newsletter subscription form, you can't ask for a gender
3. Shorter forms convert better. If someone needs to fill in a form which takes 5 minutes and he/she needs to provide information which is not relevant to request and objective of the form, the visitor perhaps will not complete our call to action and we lose a potential lead

## **Three standard forms**

We have create **three standard forms** that you can use as a starting point:

1. Form template - Sign Up
2. Form template - Short
3. Form template - Full

## **1. Form Template - Sign up**

This template has only one field, an email address. This can be used for newsletter subscription forms and for example a download request. In these cases all the information we need is the email address to process the request (like adding the person to the newsletter).

**Field**

- email address

## **2. Form Template - Short**

This template has 3 fields, first name, last name, company name and email address.

**Fields**

- first name
- last name
- company name
- email address

## **3. Form Template - Full**

This template has most fields like first name, last name, company name and email address, phone number and detailed address information (Street, street 2, city, state, ZIP/Postal code and country.

**Fields**

- first name
- last name
- company name
- email address
- phone number
- detailed address (street, street 2, city, state, Zip/Postal code and country)

These templates are available on all websites. Copy the template form and change the name to your new form. Always use the shortest form to fulfill the call to action.

**Important note: always add in the Form Title or Mail Subject the name of the website.**

**Let's say you manage multiple sites which all have a newsletter. Then, if you receive a mail with the title "New Newsletter Subscription" you don't know from which site the form comes ands where you have to add the new subscriber. If you add the site name in the title it saves a lot of time as you know that "New Newsletter Subscription GreenSteelWorld.com" comes from GreenSteelWorld.com.**
